OP42 defect: `quran_layer_dataset.json → _meta.distribution_total: {Madani: 180, Makki: 180}` — a sample split misread as the true occurrence distribution.

D2 `quran_layer_dataset.json → _meta.distribution_total: {Makki: 53, Madani: 4}` — unchanged, correctly uses the field for total occurrences (57) ✅ CONSISTENT.

The fix went beyond a field value change: the field name itself was renamed from the ambiguous `distribution_total` to the self-documenting `sampled_refs_by_place`, and the authoritative numbers were added as a separate field with an explicit source citation. Mission notation "renderer fixed permanently" confirmed — the build code now distinguishes the two concepts at source. RX2 **CLOSED**.

OP42 defect: 18:10 absent from SOURCES.md §1 table; layer has `unique_ayah_keys: 42`.
